 What You Need to Know:  

• Please report 45 minutes in advance of your appointment time for x-rays.

• Bring all insurance cards for us to copy and be prepared to pay any office visit co-pay at time of service.

• Please bring your authorization/referral for this visit.

• Complete the enclosed questionnaire and bring it with you to your visit.

• Minors must be accompanied by parent/legal guardian.

• Adults who cannot sign their own Consent for Treatment must be accompanied by Conservator and legal documents verifying Conservatorship.

• Please bring past diagnostic studies (x-rays, MRI, CT scan, etc) which we discussed when making your appointment.  DO NOT MAIL YOUR STUDIES.  You must hand carry them to your visit.
